Neighborhood Intervention Team

The Neighborhood Intervention Team is made of five Community Health Integrators who work in the community to increase awareness of type 2 diabetes and reduce serious complications from the disease.

The Neighborhood Intervention Team works with community partners to:

  • Link people to available diabetes resources,
  • Assist with health fairs, 
  • Give presentations and
  • Teach diabetes self-management and chronic disease self-management classes.

The Community Health Integrators help coordinate annual events such as Diabetes Alert Day and Take a Loved One to the Doctor.

The goal of all of these efforts are to improve the health outcomes of individuals living with type 2 diabetes and find those living with
type 2 diabetes but don't know.
