The Durham Diabetes Coalition has three teams working together to make an impact on type 2 diabetes in Durham County.

Services that DDC provide include:

• Linking patients to community and diabetes resources,
• Providing home visits to patients who qualify,
• Analyzing data,
• Making maps to see patterns related to health and geography,
• Teaching diabetes and chronic disease self-management classes and
• Helping communities take charge of their health.
