Clinical Services Team

The Durham Diabetes Coalition has two teams working to improve the health and quality of life of adults in Durham County living with type 2 diabetes. The teams link referred patients to community resources. Some patients receive home visits from the team.

Each Clinical Team has a:

  • Supervising Physician who supervises the work of the team,
  • Nurse Practitioner who examines patients, works with the patient's primary care doctor and provides follow-up care,
  • Licensed Social Worker who finds out patient needs, provides case management and counsels some patients,
  • Registered Dietitian who discusses dietary needs and helps develop a nutrition plan and a
  • Community Health Worker who links patients with community resources and helps schedule appointments.

Patients must be referred by
their doctor and meet certain guidelines in order to receive
home visits from the Clinical Team.
