Chronic Disease and Diabetes Self-Management Classes

A FREE 6-week program that helps people manage and improve their own health, while reducing health care costs.

Participants will learn about pain management, nutrition, exercise, appropriate use of medication, emotions, communicating with doctors. Participants will make weekly action plans, share experiences, and help each other solve problems they encounter in their self-management program. The diabetes-specific classes will focus on people living with diabetes and those who care for them.
